Flexible services for individual requirements

We have dedicated support coordination staff as well as a team of support workers who are ready to assist you every step of the way.

We understand that everyone has different needs, so we always strive to provide flexible services tailored towards individual goals and preferences.

Support coordination helps you to get the best use of your supports in your NDIS plan. It helps you to:

  • Understand and utilise your NDIS plan to achieve your goals.
  • Connect with NDIS providers, community resources, mainstream services, and other government programs.
  • Enhance your confidence and skills to utilise and coordinate your supports effectively.

All the above can be achieved by connecting you with a designated Support Coordinator, who will deliver the support coordination services, you require as part of our plan so you can meet your goals.

There are three levels of support coordination that can be included in your plan:

  • Support connection – This support aims to enhance your ability to connect with informal, community, and funded supports, helping you maximise your plan and achieve your goals.
  • Support coordination – coordination of supports: This support will help you develop the skills needed to understand and utilise your plan effectively. A support coordinator will collaborate with you to ensure a balanced mix of supports, enhancing your ability to maintain relationships, manage service delivery tasks, live more independently, and participate in your community.
  • Specialist support coordination – This higher level of support coordination is designed for individuals with more complex situations who require specialist assistance. A Specialist Support Coordinator will help you navigate challenges in your support environment and ensure consistent delivery of services.
